Enrollment is not complete without a deposit. See forms for prices. A Health Inventory Form (Medical Physical) & immunizations are required for enrollment for camp and for school year care. A hardcopy emergency form is available as an option to submitting it online. The health inventory and the emergency form are due the first day of enrollment.
